I hereby acknowledge that:

1. the data controller required to ensure lawful processing of my personal data is the University of Wrocław (Uniwersytet Wrocławski), pl. Uniwersytecki 1, 50-137 Wrocław;
2. the data controller has appointed the data protection inspector, who can be contacted via e-mail: IOD@uwr.edu.pl;
3. my personal data shall be processed for the purpose of the recruitment procedure for accommodation in the halls of residence (by separate agreement) and for archival and statistical purposes. The legal basis for the processing: my consent and the act Law on Higher Education and Science of 20 July 2018 (Journal of Laws of 2021, item 478);
4. submitting my personal data is voluntary, and if I refuse to provide them, my application for accommodation may not be considered;
5. my personal data shall not be shared with any external entities, except for the cases set out in the provisions of law, in particular in the act Law on Higher Education and Science of 20 July 2018 (Journal of Laws of 2021, item 478) or agreements or contracts concluded by the University of Wrocław; If I receive a place in a student residence and I give my consent, my name will be placed on the list of persons accommodated in a specific student residence, published on the University of Wrocław public pages;
6. my personal data will be kept for the time necessary to fulfil the purposes set out in point 3, taking into account applicable legislation on filing systems; this does not apply to consent to publication on the list of persons accommodated referred to in point 5;
7. I have the right to withdraw my consent that constitutes the legal basis for the processing, in any time, without any effect on the lawfulness of the processing performed before my consent was withdrawn; however, withdrawing my consent is equivalent to terminating my participation in the recruitment process;
8. I have the right to access, correct, transfer, and delete my personal data, limit their processing and object to it. However, I am aware that my rights may be limited by specific regulations;
9. decisions in my case shall not be made in an automated manner, and the personal data shall not be subject to profiling;
10. I have the right to lodge a complaint with the president of the Office of Personal Data Protection.

I hereby acknowledge that the University of Wrocław shall not bear any responsibility for any error I make when entering my data in the registration system for accommodation in the halls of residence.
